This might involve conversation, facilitation, education, practice development, coaching or simply creating the space to think together. The work happens where life happens: at waking, dressing, mealtimes and settling at night, on day and night shifts alike. Staff explore how everyday routines can offer residents more choice, stronger connection and a calmer, more predictable day.

Progress isn't measured simply by the number of conversations held or activities completed. Together, we regularly pause to consider changes in leadership practice, everyday interactions, resident and staff experiences, and how far wellbeing is becoming part of the culture and routines of the community.

Different contexts. Different ways of working.

There's no standard true2you package. Most partnerships begin in one of three ways:


Community Wellbeing Partnership — for new communities
Intentionally shaping a community's culture during development is still unusual, so the work begins with conversation — often several — to explore what's possible. Shaping a shared vision is often part of this early stage. That vision then guides the work that follows, which may include:

  • Guidance on leadership appointments

  • Support in selecting service providers whose ways of working reflect the community's vision

  • Designing operational systems that support person-directed practice from day one

  • Leadership development and reflective practice as the community takes shape

Each year's priorities are set together in an Annual Programme Plan.


Community Wellbeing Partnership — for established communities
Your community already has strengths, and this journey builds on them. It begins on site with relationships rather than an audit: listening to residents, families, staff and leaders, and observing everyday life. The first twelve months typically include:

  • An initial 2–3 day on-site visit

  • Monthly online leadership conversations

  • Individual development conversations with leaders

  • Quarterly on-site practice development visits

  • Ongoing support between visits

  • An annual Appreciative Community Wellbeing Review


Starting with a team
Some communities begin with a focused journey with one team, for example nursing and senior staff in a care centre or a home for people living with dementia. Practice sessions are built around daily routines, and the journey can grow into a wider Community Wellbeing Partnership over time.

From there, we prepare an overview tailored to your community. If you decide to go ahead, we develop the first-year plan together.
